At Sojourn at Seneca Senior Behavioral Health Hospital, we understand that the admissions process can be overwhelming. Our goal is to make it as smooth and straightforward as possible, ensuring that you or your loved one receives the care they need promptly and efficiently.

Admissions Criteria

Sojourn at Seneca provides short-term inpatient psychiatric care for seniors aged 55 and older who need stabilization in an acute care setting. Our facility is equipped to treat a variety of conditions, including:

  • Depression

  • Suicidal/homicidal thoughts

  • Paranoia

  • Delusions and/or hallucinations

  • Behavioral disturbances

  • Alzheimer’s and Dementia

  • Anxiety

  • Schizophrenia

  • Bipolar Disorder

  • Insomnia and/or changes in sleeping patterns

  • Increased forgetfulness or confusion

  • Acute onset of inability to care for self or attend to activities of daily living

  • Refusal to take medications

 

 

Referral Process

Step 1: Initial Contact

  • Call the 24-Hour Referral Line: 567-207-2230

  • Provide the following information:

    • Demographic information

    • Brief description of presenting behavioral issues or symptoms

    • Insurance information

 

Step 2: Submit Referral Packet

  • Fax: 567-245-4587

  • Email: info@sojournseneca.com

  • Include the following documents:

    • Admission Face Sheet, Insurance Information, and Demographics

    • Advance Directives, Medical Power of Attorney, or Guardianship Papers (if applicable)

    • Current progress notes documenting signs and symptoms

    • History and Physical

    • Psychiatric assessment or relevant consults

    • Current Medication Administration Record (MAR) or medication list

    • Current Labs: CBC, BMP, and UA

 

Step 3: Review and Admission

  • Patient information will be promptly reviewed with the psychiatrist to determine if admission is appropriate and to ensure criteria have been met.

  • Our intake coordinator will contact you with the disposition.

  • The referring facility will arrange for transportation.

  • A negative COVID test is required prior to admission.

  • A pink slip must be completed prior to admission (if applicable).

  • A nurse-to-nurse report must be conducted prior to transport.

The length of stay varies based on the individual needs of each patient and their treatment plan. Our goal is to stabilize and support patients so they can return to their normal living environment as quickly as possible.

 

Can family members visit?

Yes, family involvement is encouraged. Please refer to our Visiting Hours page for details on visitation policies and scheduling.

 

What should I bring for the patient?

We recommend bringing personal hygiene items, comfortable clothing, and any necessary medical documents. Please note that personal belongings such as cellphones and purses are not allowed on the unit and will be securely stored.
